Description

The Alzheimer's Disease Programs Initiative (ADPI) - State and Community Grant Program, administered by the Administration for Community Living (ACL), supports the development and expansion of dementia-capable home and community-based service (HCBS) systems. This funding opportunity provides cooperative agreements to help individuals living with dementia and their caregivers remain independent and safe in their communities.

Overview

This Notice of Funding Opportunity (NOFO) includes two application options: Grants to States (Option A) and Grants to Communities (Option B). No entity is eligible to apply for both options, and no entity may hold more than one ADPI grant at a time. The resulting dementia-capable systems are expected to deliver quality, person-centered services and supports.

Eligibility

Option A – Grants to States: Eligible applicants are governmental entities within states and territories designated as the state agency for dementia-capability. States and territories without active ACL ADPI State dementia-capability grants may apply. Priority consideration will be given to states that have not benefited from ADSSP grants since before 2014.

Option B – Grants to Communities: Eligible applicants are private or public community-based organizations (CBOs) that operate within an existing dementia-capable HCBS system. CBOs without active ADPI grants may apply. Priority consideration will be given to organizations that have not benefited from ADI-SSS and ADPI grant programs since before 2014.

Eligible applicant types include:

  • State governments
  • County governments
  • City or township governments
  • Special district governments
  • Federally recognized Native American tribal governments
  • Native American tribal organizations
  • Nonprofits with 501(c)(3) status (non-higher education)
  • Private institutions of higher education
  • Public and state institutions of higher education

Foreign entities are not eligible. Faith-based and community organizations that meet eligibility requirements may apply.

Research Areas and Activities

Option A – Grants to States (36-month cooperative agreements): Applicants must address two required objectives:

  • Creation, expansion, and sustainability of a dementia-capable state HCBS system that includes Single Entry Point/No Wrong Door (SEP/NWD) access for people with dementia and their family caregivers.
  • Ensuring access to a comprehensive, sustainable set of quality state HCBS that are dementia-capable and provide innovative services.

Option B – Grants to Communities (36-month cooperative agreements): Applicants must address all three of the following service gaps:

  • Provision of effective supportive services to persons living alone with Alzheimer's disease and related dementias (ADRD) in the community.
  • Improvement of the quality and effectiveness of programs and services for individuals aging with intellectual and developmental disabilities who have ADRD or are at high risk.
  • Delivery of behavioral symptom management training and expert consultations for family caregivers.
