Opportunity Description

This opportunity supports the development and implementation of programs aimed at improving health outcomes for older adults through chronic disease management and fall prevention.

The selected entity will be responsible for:

  • Developing evidence-based curricula for chronic disease management (e.g., diabetes, hypertension).
  • Implementing community-based fall risk assessment and prevention programs.
  • Conducting needs assessments within target communities to tailor interventions.
  • Evaluating the effectiveness of the implemented programs using established metrics.

Successful applicants must demonstrate prior experience in geriatric care, public health program management, and community outreach.

Eligibility and Requirements

Eligibility

Applications are open to non-profit organizations, academic institutions, and community health centers that serve populations aged 65 and older.

Required Documentation

  • Proof of 501(c)(3) status (or equivalent).
  • Three letters of support from local healthcare providers.
  • A detailed organizational budget and sustainability plan.

Key Focus Areas

The project must address at least two of the following areas: Polypharmacy management, social isolation, nutrition, and physical mobility.
