The Aged Care Services - Specialist Dementia Care Program supports the provision of specialized, transitional residential support for individuals exhibiting very severe behavioural and psychological symptoms of dementia who cannot be appropriately cared for by mainstream aged care services.
The Specialist Dementia Care Program (SDCP) is an Australian Government initiative. The program focuses on reducing or stabilizing symptoms over time, with the ultimate aim of enabling individuals to transition to less intensive care settings. The intention is for 35 SDCP units to be established, with one unit planned for each of the 31 Primary Health Network (PHN) regions. To date, 10 SDCP units have been established nationally across 9 PHN regions through Phase One Grant Opportunities. This Phase Two SDCP Grant opportunity seeks to identify aged care providers with the capacity to establish the remaining SDCP units.
