Description

The Chronic Conditions Prevention and Integrated Care Program (GO8398) supports improved health outcomes for people in Australia living with, or at risk of, chronic conditions by enabling early detection, prevention, and effective management. Guided by the National Strategic Framework for Chronic Conditions 2026-2035, the Program provides a strategic and competitive investment approach that ensures transparency, fairness and alignment with Government priorities.

Overview

Funding will be awarded to initiatives that demonstrate measurable impact and contribute to long-term, sustainable improvements in chronic conditions health outcomes. The objectives of the Program are:

  • Health Promotion and Education
  • Embedding Prevention and Early Detection
  • System integrated, multidisciplinary and person centred continuity of care
  • Managing Multimorbidity
  • Enhanced and targeted support for health equity

Funding and Benefits

Total Amount Available: AUD $148,235,000.00

Funding is prioritised under two tiers:

  • Tier 1: National Collaborative Activities – Supports large-scale activities with national scope that address multiple chronic conditions and/or risk factors and meet at least two Program objectives. These initiatives foster cross-sector collaboration and use a unified approach addressing related conditions or common risk factors regardless of specific diagnoses.
  • Tier 2: Targeted Activities – Supports short‑term activities that contribute to at least one Program objective. These activities may focus on a single chronic condition or risk factor and can include pilot projects and time‑limited trials.

Eligibility

To be eligible, an applicant must be one of the following entity types:

  • Aboriginal and/or Torres Strait Islander Corporation registered under the Corporations (Aboriginal and /or Torres Strait Islander) Act 2006
  • Company
  • Cooperative
  • Incorporated Association
  • Statutory Entity
  • Partnership

If applying as a Trustee on behalf of a Trust, the Trustee must have an eligible entity type as listed above. Applications from consortia are encouraged, provided a lead organisation that is solely accountable to the Commonwealth for delivery of grant activities and is an eligible entity per the list above. Only the lead organisation may submit the application form and enter into a grant agreement. Eligible organisations may form a consortium with ineligible organisations, except those included on the National Redress Scheme’s list of ‘Institutions that have not joined or signified their intent to join the Scheme’.

Applications are also accepted from the following entities:

  • Non-Government Organisation
  • Primary Health Networks
  • Community Organisations
  • Academic Institutions

Application Process

Applicants must submit the completed application form and all necessary attachments by the closing date and time via the Online Application Form, accessible through the “Grant Opportunity Documents” button on the left-hand side of the opportunity page. A copy of the application and any supporting documents should be retained. An automated email notification acknowledging receipt of the submission, including a Submission Reference ID, will be sent. Retain this ID to query the application. If no automated email notification is received within 48 hours of submission, contact Grant.ATM@health.gov.au.

Other Instructions: Attachment size is limited to 2 MB per file in the application form. Multiple files or documents should be scanned into a single document. Compressed or zip files are not accepted. File names must be unique and must not include foreign characters.
