Application Process
- Phase 1: Letter of Intent (Due Date: October 15th)
- Submit a 2-page summary outlining community need and proposed activities.
- Must include letters of support from at least two local healthcare providers.
- Phase 2: Full Proposal Submission (Due Date: November 30th)
- Detailed budget breakdown and M&E plan.
- Resumes of key personnel.
